• Make sure your employer has completed a Limousine Driver or Rental Car Chauffeur Employer Certification Application [pdf] for you and mailed it to MVC
• Review the Limousine Driver letter
• Complete the fingerprint Application [pdf] and bring it to your scheduled appointment along with proper identification
• Schedule your appointment online or call toll-free
  (877) 503-5981 to make an appointment by phone. You must pay at time of scheduling with a credit card, debit card or money order
• There will be a $50 fee incorporating all background checks and vendor fees
• Payment must be made with a credit card, debit card or money order at time of scheduling
